Coaching Coach/Coachee Agreement

This agreement is between: Dr Cindy Croucher-Wright (Coach) and ___________________ [name].
Whereby the Coach agrees to provide Coaching Services for: ________________________

Description:
Rather than dealing with the treatment of disease, a health coach focuses on wellness and the prevention of illness through promoting a healthy lifestyle to achieve optimal health. A health coach primarily coaches, educates and motivates coachees in a thought-provoking, empowering, and creative process that inspires the coachee to bridge the gap between their current health and their goals by assuming more personal responsibility for their health and by adopting a healthy attitude, lifestyle, and diet. A health coach helps coachees achieve their health and nutrition goals through behaviour change techniques, motivational interviewing, and other activities.
Dr Croucher-Wright is not a medical doctor, dietitian, nutritionist or psychologist, and the scope of the consultation services does not include treatment or diagnosis of specific illnesses or disorders. Health coaching does not and nor is it intended to replace a consultation with a doctor, nurse, or other appropriately qualified practitioner. Dr Croucher-Wright, does not accept responsibility for any loss, damage or injury that arises from this service. If the Coachee suspects they may have an ailment or illness that may require medical attention, then they must consult with an appropriately qualified medical practitioner.
Responsibilities:

The Coach respects and agrees to abide by the UKIHCA Code of Conduct.

The Coachee is responsible for creating and implementing his/her own physical, mental, and emotional well-being, decisions, choices, actions, and results. As such, the Coachee agrees that the Coach is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Coach. The Coachee understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.

The Coachee understands that coaching is not to be used as a substitute for professional advice by legal, mental, medical, or other qualified professionals and will seek independent professional guidance for such matters. The Coach will not be held liable for failure to diagnose or treat a disease, nor will he/she be liable for failure to prevent future illness.

The Coachee agrees to communicate honestly, be open to feedback and assistance and create the time and energy to participate fully in the program.
Services:
The parties agree to engage in a Weekly Coaching Program online. Sessions will last 60-90 for the first meeting and 30-45 minutes for subsequent meetings. The Coach will be available to Coachee by e-mail and WhatsApp in between scheduled meetings. The Coach may also be available for additional time/coaching sessions/Coachee related service outside of coaching hours, per Coachee’s request charged on a prorated basis as stated in the fees below.
Schedule & Fees:
This coaching agreement is valid as of __________ (DD/MM/YY.) The fee is £60 per session and is payable in advance of either a 4-week or 6-week package. (Individual sessions are charged at £60/hour on pro-rated basis as needed). If the rates change before this agreement has been signed and dated, the prevailing rates will apply.
Procedure
The time of the coaching meetings and/or location will be determined by Coach and Coachee based on a mutually agreed upon time. The Coachee will initiate all scheduled calls and will call the Coach on 07932 977 999 for all scheduled phone meetings, or online via a Teams call.
Confidentiality:
This coaching relationship, as well as all information (documented or verbal) that the Coachee shares with the Coach as part of this relationship, is bound to confidentiality as defined by the UKIHCA Code of Conduct. The Coach agrees not to disclose any information pertaining to the Coachee without the Coachee’s written consent.
Cancellation and Lateness Policy:
The Coachee agrees that it is their responsibility to notify the Coach at least 24 hours in advance of the scheduled call/meeting if they cannot make the session. The Coachee will be charged for a missed session if they do not turn up or reschedule. The Coach will wait for 15 minutes, before the Coachee is noted as not turning up for the session.
Termination:
Either the Coachee or the Coach may terminate this agreement at any time with 2 weeks written notice.
